Cannot find user xxx in the directory

Is there anything wrong with my configuration? I have created the active directory account as follows:
and i try to login using this account with LDAP/AD Login for intranet, and here is this my config:
but when i try to Test Authentication here is this i got the result
i really don’t know what’s wrong with my configuration. I already try with (&(objectClass=*)(cn=?)) and (&(objectClass=*)(sAMAccountName=?)) but i got the same result.
i hope somebody can give me clue
